This kit contains two letters to be used to challenge medical collection accounts reporting on your credit reports.

Letter 1- HIPAA Validation Letter: Send this letter directly to the collection agency for full validation of any medical collections on your credit report .

• Do not sign. Type name only. Do not include id.

Letter 2- HIPAA Violation Letter: Send this letter directly to the collection agency and the credit bureaus if the collection agency does not cease collection in response to the validation letter. Letter can be sent to both the collection agency and the credit bureaus on the same day.

• Do not sign. Type name only. DO NOT include id for letters to collection agency. ALWAYS send id and proof of residency with letters to credit bureaus.
